To undertake a WWCC new transaction we need to get your consent to a number of things:
1.I consent to the Department of Justice and Community Safety (the department), its agents, contractors and their subcontractors receiving, sharing, copying and processing the information in this application and its attachments for the purposes of the Working with Children Act 2005 (the Act) and the Working with Children Regulations 2016 (the Regulations).2.For the purposes of this application, and if I am granted a Working with Children Check (Check), for ongoing enquiries while I continue to hold the Check, I consent to:a.the department, seeking information about me from any source considered necessary, including any Australian Police agency, the National Police Checking Service known as the Australian Criminal Intelligence Commission (ACIC), any court, prosecuting authority, authorised screening agency, Professional Registration Board, relevant prescribed body, health treating professional, agency and any government department including the Department of Health and Human Services, Commission for Children and Young People (CCYP) and Corrections Victoria; andb.the disclosure of information about me by the department to any person or organisation considered necessary, including any Australian Police agency, the National Police Checking Service known as ACIC, any court, prosecuting authority, authorised screening agency, Professional Registration Board, relevant prescribed body, health treating professional, agency and any government department including the Department of Health and Human Services, Commission for Children and Young People (CCYP) and Corrections Victoria.3.I acknowledge that the information which may be disclosed to the department includes, but is not limited to, the details about or circumstances relating to, convictions, findings of guilt and charges regardless of the outcome of the charges or when and where the offence or alleged offence occurred.4.With respect to the ACIC National Police Checking Service I acknowledge that:a.the department is collecting my personal information to check if I have a criminal history and assess whether I should be permitted to work with children;b.the department provides my personal information to ACIC and the Australian police agencies;c.ACIC discloses my personal information to the Australian police agencies;d.the information released as part of the National Police History Check is done so in accordance with spent convictions legislation/information release policies1; e.in applying for a Check, there may be exclusions from spent convictions legislation/information release policies that apply;f.the Australian police agencies may be disclosing to ACIC information from their records and details of convictions and charges including findings of guilt or the acceptance of a plea of guilty by a court – the information is disclosed in accordance with the laws of the Commonwealth, states and territories and, in the absence of any laws governing the disclosure of this information, disclosing in accordance with the polices of the police agency concerned;g.ACIC provides the information disclosed by the Australian police agencies to the department in accordance with the laws of the Commonwealth; h.any information provided in my application for a Check is provided for the purpose of obtaining the required Check under the Act and the Regulations;i.any information provided by the Australian police agencies or ACIC is provided specifically for the purpose of obtaining the required Check under the Act and the Regulations;j.it is usual practice for an applicant's personal information to be disclosed to Australian police agencies for them to use for general law enforcement purposes, including the investigation of any outstanding criminal offences and any other purposes permitted in the Australian Crime Commission Act 2002 (Cth);k.I may withdraw my consent at any time by sending the department written notice addressed to:Working with Children Check Victoria

MELBOURNE VIC 3001 l.the information I provide as part of my application for a Check, which ACIC provides to the department on receipt of the application for a Check, will be used only for the purpose stated above unless statutory obligations require otherwise; m.if I have any questions or wish to dispute the result of my police check I will contact Working with Children Check Victoria using the details provided in paragraph k above;n.if I do not want to discuss the outcome of my Police Check with Working With Children Check directly that I can contact the department’s privacy officer on 8684 0178 or by email: email@example.com.If the CCYP notify the department at any time of a finding of reportable conduct against me pursuant to 16ZD of the Child Wellbeing and Safety Act 2005, I consent to the department notifying CCYP of the outcome of any application or re-assessment assessing my eligibility to engage in child-related work. If I am given a negative notice and make an application to the Victorian Civil and Administrative Tribunal, I consent to the department notifying CCYP that I have made such an application.6.I acknowledge that the Act and Regulations require that my Personal Information be collected and the consequence of non-compliance is that my application for a Check will not be processed.7.I acknowledge it is an offence to provide false or misleading information in a material particular in relation to an application for a Check or in connection with a re-assessment under the Act, subject to a maximum of 2 years imprisonment or a level 7 fine or both.8.I acknowledge it is an offence to apply for a Check if I am subject to an order under the Sex Offenders Registration Act 2004, Serious Sex Offenders Monitoring Act 2005 or Serious Offenders Act 2018 liable to a penalty of 240 penalty units or imprisonment for 2 years.9.While my application is being assessed, I understand that if my criminal record discloses charges, offences or orders specified in Schedule 3 of the Act, regardless of when and where they occurred, I must not do paid or volunteer child-related work. I consent to the department notifying the organisations whose details I have provided to the department that I am prohibited from engaging in paid or volunteer child-related work while my application is being assessed. I also understand that the 'Check status' service on the Check website during the course of the assessment will indicate that I am ineligible to do paid or voluntary child-related work.10.I understand that in accordance with the Act and the Regulations, the department may disclose personal information about me to the organisations whose details I provide to the department, or where otherwise permitted by any act or regulations.11.I understand that I must notify the department within 21 days of changes to my details. This includes my personal and contact details and the details of all organisations where I am doing child-related work.12.I consent to the department correcting contact information in respect of the organisations for which I am doing child-related work, if it is satisfied that the information held is incorrect.13.I understand that the Act requires that I notify the department and any organisations where I am doing child-related work within 7 days in certain circumstances, including if I am charged with or found guilty of certain offences, if I become subject to reporting obligations imposed by Part 3 of the Sex Offenders Registration Act 2004, an extended supervision order or interim extended supervision order under the Serious Sex Offenders Monitoring Act 2005, a supervision order, detention order or an emergency detention order, or if a relevant finding is made against me by a prescribed body.14.I consent to the department:a.using the contact details I provide to request information (including any document, written submission or consent) for the purposes of assessing or completing my application; andb.notifying me (including through Service Victoria, where authorised) about the finalisation, progress and outcome of my application and to request feedback from me relating to the Check.15.If I am issued with a Check, I consent to the department using the contact details I provide to contact me about updating my details, remind me about the expiry of my Check, send me periodic emails and eNewsletters, request information (including any document, written submission or consent) when re-assessing my eligibility to hold the Check, and notify me of the progress and outcome of any such re-assessment.
